Job Opening: Community Health Worker at South Cumberland Community Clinic

LIFT Grundy is much more than giving people rides (though we are a mobility network). We are also in the business of helping people to move toward healthier and more prosperous lives. Key to that work is our Community Health Workers, who accompany people who are on a journey to better health, connecting them with resources and supporting them as they manage their health challenges.

Community Health Worker Position

We are seeking a Community Health Worker to be based at the South Cumberland Community Clinic. In this role, you will connect people to services, help them navigate healthcare and other systems, and support access to basic needs like food, housing, and transportation. This position includes work in both clinical and community settings and collaboration with clinic staff and local partners.

This role is part of the LIFT Grundy Health Mobility Project and includes participation in a CHW apprenticeship program, offering hands-on training and a pathway to certification in Tennessee.
