Community Health Resources

Community Health Workers are currently serving at the South Cumberland Community Clinic in Tracy City, The Beersheba Springs Medical Clinic, and with the Southeast Tennessee Human Resources Agency (SETHRA). 

A community health worker (CHW) is a front line public health worker who is a trusted community member. They build relationships with patients and connect health and social services in the community. CHWs help patients achieve health goals by addressing each client's unique needs outside the clinic setting.

People served include individuals with barriers to health or social service support and individuals struggling with chronic disease self-management by providing resources and connections with housing, transportation, state benefits, food, health promotion and coaching, lifestyle and social assistance, health system navigation, health education, and more.
