Community Health Resources

Community Health Workers (CHWs) help people connect with health care, community services, and other support they may need.

LIFT Grundy Community Health Workers serve people across Grundy County through partnerships with South Cumberland Community Clinic, Beersheba Springs Medical Clinic, and SETHRA. CHWs can meet with people at their partner sites, in the community, or by appointment at the Littell-Partin Center.

​

A CHW can help with things like transportation, food or housing resources, benefits and assistance programs, health care, referrals, and other local services. They can also help people work toward health goals and figure out what steps to take next.
